Find the 18-digit survey code at the bottom of your receipt, as shown in the below image.

Share your shopping experience and answer the questions of Lowe’s survey.

Once the online survey is done, you must enter a $500 monthly sweepstakes program.

You need to submit your contact details to the survey administration.

• The participant can only choose two ways to enter: By online survey or Mail-In entry.
• Only United States and Washington, D.C. residents are permitted.
